The Misdiagnosis of Squamous Cell Carcinoma of Lacrimal Sac as Nasolacrimal Duct Obstruction

Abstract
Purpose: The incidence of lacrimal sac tumor is rare and it is not easy to rule out the possibility of lacrimal sac tumor in patients with symptom of chronic dacryocystitis and epiphora. We report the case in which the patient with lacrimal sac squamous cell carcinoma was misdiagnosed as simple nasolacrimal duct obstruction and underwent endonasal dacryocystorhinostomy.

Methods: A 56-year-old male was underwent endonasal dacryocystorhinostomy with silicone tube insertion in private clinic and presented with dacryocystitis and orbital cellulitis of unknown cause on postoperative 3rd week. After systemic antibiotic therapy and removal of silicone tube, then he was transferred to our hospital, nasal mucosa biopsy was resulted in squamous cell carcinoma and paranasal sinus CT showed that infiltration of tumor into ethmoidal and maxillary sinus. Our otolaryngology department did radical excision including orbital exenteration, removal of lacrimal sac, nasolacrimal duct, etc. Then radiotherapy was added.

Results: The patient has been under regular follow-up for 12 month after operation and until recently there has been no evidence of distant metastasis and local recurrence in view of imaging study and clinical examination.
